Your Application Checklist

• A Completed Application
• Personal Photo
• 750 Word Essay
• Three References
• Official High School Transcript must include SAT or ACT Scores
• Copy of college acceptance letter(s)
• Signed 2009 Tax Information Sheet for Parental/Guardian (Usually 1040 or 1040A form)
• W2 Forms (if self-prepared)
• All necessary tax return schedules attached
• Parental signature(s) on all tax forms
• Preparer’s signature and name of firm (if professionally prepared)
